Minimum Qualifications Permanent Transfer Candidates : Current NYS Department of Health employee with one year of permanent or contingent-permanent, competitive, status as a Health Systems Specialist 2 (Social Work) SG20.
Non Competitive Candidates Via NY HELPS : Possession of New York State license and current registration as a licensed master social worker, or licensed clinical social worker AND one year of post- licensure experience providing clinical services in a licensed healthcare facility or program, or utilizing clinical knowledge in the review and oversight of clinical services for quality assurance or reimbursement purposes;
or possession and licensure as a Licensed Clinical Social Worker.
Preferred Qualifications : Ability to evaluate records against applicable laws and regulations. Ability to draft and / or review Statements or Notices of Deficiencies, Plans of Correction, Investigative reports, and survey information against statewide standards for consistency and quality assurance.
Ability to gather data as needed for reports, recommend enforcements or sanctions. Strong organization skills and proficiency with the Microsoft Office product suite.
Excellent oral and written communication skills. Federal Surveyor Minimum Quality Test certification and investigatory experience is a plus.
Duties Description With an eye toward continuous quality improvement, serve the Regional Office team as the Nutritionist for the adult care facility surveillance program.
In this capacity, the incumbent will participate on a multi-disciplinary health and safety survey team as the chief inspector of facilities’ dietary compliance, assessing State and Federal regulatory compliance through onsite surveys or independently depending on the nature of the assessment.
In addition, the incumbent will participate in emergency preparedness and response activities, provide technical support and assistance to facilities where appropriate, draft inspection reports and review same drafted by lower-level staff to ensure regulatory compliance, assessing This will require that the incumbent meet with dietician teams at adult care facilities and assisted living programs and conduct interviews of residents to assess overall compliance as needed to drive quality of life improvements in such facilities.
In addition, the incumbent will prepare well-written reports related to their findings, participate in interdisciplinary reviews as needed, and provide constant feedback about necessary improvements to impact the suitability of aged and aging services.
