Manager of Intake (Registered Nurse) for Home Health

Job Summary : The Manager of Intake is responsible for overseeing the intake process for new patient referrals and admissions within a home health company.
This role involves managing a team that processes patient referrals, ensuring clinical appropriateness, verifying insurance coverage, and coordinating with various healthcare providers to facilitate smooth onboarding.
This position requires a Registered Nurse (RN) credential due to the clinical assessment needs and understanding of home health regulations, including Medicare requirements.
Key Responsibilities :
1. Intake Process Management
- Lead and oversee the intake team to ensure timely processing of patient referrals.
- Review and assess each referral for clinical eligibility and appropriateness for home health services.
- Establish and maintain policies and protocols for the intake process to ensure compliance with healthcare regulations.
2. Clinical Oversight and Quality Assurance
- Evaluate patient care needs in coordination with the clinical team, ensuring the company can meet the specific care requirements of each referral.
- Provide clinical guidance to intake team members on identifying appropriate care plans and service offerings based on patient conditions.
- Ensure all intake records are accurate, complete, and compliant with state, federal, and company standards.
3. Coordination with Healthcare Providers and Case Managers
- Act as the primary point of contact for referral sources, including hospitals, physicians, and case managers.
- Collaborate with care coordinators to facilitate a seamless transition from hospital or facility to home care.
- Liaise with insurance providers to verify coverage and address authorization requirements for services.
4. Team Leadership and Development
- Train and mentor intake staff, including nurses and administrative personnel, to understand home health protocols, regulations, and patient care standards.
- Conduct regular performance evaluations, provide feedback, and implement professional development initiatives to enhance team skills.
5. Data Reporting and Process Improvement
- Monitor and report on intake metrics such as referral conversion rates, patient satisfaction, and response times.
- Implement continuous improvement initiatives to optimize the intake workflow and enhance the patient experience.
Qualifications :
- Education : Active RN license in good standing. BSN preferred.
- Experience : Minimum of 5 years of nursing experience, preferably in home health or a related field, with at least 2 years in a supervisory or management role.
- Skills :
- Strong understanding of Medicare regulations, home health standards, and insurance authorization.
- Excellent communication skills for interacting with patients, families, referral sources, and internal teams.
- Proficiency in healthcare software for managing patient information and intake records.
