The Clinical Pharmacy Specialist (CPS) will collaborate with primary care physicians and staff to provide coordinated, comprehensive, high-quality care to PMG patients.
The responsibilities of the Clinical Pharmacy Specialist predominantly fall into the following areas : (1) direct patient care, (2) population-based disease management, (3) preceptor / educator, (4) pharmacy support, (5) research / projects.
Required Qualifications :
- PharmD from an accredited college or school of pharmacy.
- Completion of a PGY-1 residency.
- Completion of a specialty / PGY-2 residency in ambulatory care or 3 years experience in primary care pharmacy (or related area) involving direct, collaborative patient care.
- Oregon Pharmacist License upon hire.
- Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
- Experience precepting students and / or residents.
Preferred Qualifications :
BPS Board Certified Pharmacist within 2 years of hire.
