MAJOR FUNCTION :
The Heart Failure Program Manager (HFPM) will serve as the lead role in the development and implementation of a system-wide Heart Failure program and assure compliance with the standards of the American College of Cardiology (ACC) to achieve accreditation designation and subsequent redesignation.
The HFPM is responsible for creating and facilitating an interdisciplinary Heart Failure committee for discussion and action planning per organizational standards.
The HFPM is responsible to monitor all steps in the Heart Failure protocol and process across the continuum of care to ensure compliance with ACC standards.
Key functions will include data analysis and reporting, writing and reviewing program policy, and supporting / completing the application process for Heart Failure Accreditation.
The HFPM must display proficient computer skills and data analysis. Excellent verbal, written, organizational, and interpersonal skills.
QUALIFICATIONS :
Education & Experience :
- BSN required.
- MSN preferred.
- Minimum3yearsacute care nursing experience required.
- Cardiovascular experience preferred, specifically congestive heart failure.
- Data abstraction and PI experience preferred.
- Knowledge of ACC accreditation requirements preferred.
Certification / Licensure :
- Active Registered Nurse (RN)in NJ required.
- National Certifications in specialty area preferred.
- BLS required.
