Overview
The Human Services Case Manager will provide case management for the Trenton Citadel social service programs under the direction of the Corps Officer, with professional supervision provided by Divisional Headquarters Human Services Staff.
The Case Manager will be able to identify the needs of the clients served and provide outreach within the community at large, partnering with outside agencies to enhance client services.
Responsibilities
- Perform intakes, interviews, and assessments of clients’ needs and perform follow-up case management with households for other supportive programs available if needed beyond emergency assistance.
- Identify and interact with case management clients; provide comprehensive case management, including but not limited to client needs assessment, developing a plan of action utilizing a strengths-based approach, and identifying internal and external programs that would support their pathway to self-sufficiency.
- Oversee clients’ cases, enter data into the client database, generate and keep a record of each case file, discuss client eligibility with Corps Officers, and keep monthly statistics.
- Coordinate services with partner agencies to ensure a holistic service approach for all clients.
- Support the Pathway of Hope Initiative by enrolling eligible families with children under 18 who are living in generational / chronic poverty and providing the above case management in conjunction with the Pathway of Hope policies and procedures.
- Ensure the accuracy and timely submission of records and reports in applicable databases for all community programs and funding sources as requested by the Corps Officers.
- Maintain accurate, confidential, and updated case records per The Salvation Army record-keeping policies and procedures.
- Assist and represent The Salvation Army at regular countywide non-profit and / or organizational meetings / events, such as Mercer COAD, National Night Out, etc.
to identify resources and referrals for individuals, share information about programs available at local Corps, and organize partnerships for programs or special events throughout the year.
- Build and maintain relationships amongst local organizations, and create partnerships for client / case referrals, special events, and seminars for community residents.
- Build, update, and maintain a directory of referral resources, contacts, and services using the client database (Service Point).
- Track and update grants associated with rental assistance and submit them to Corps Officers for review.
- Extract monthly reports from the client database to identify patterns and recommend service improvement.
- Create and maintain a community contacts resource database.
- Prepare and maintain Salvation Army statistical reports for timely submission.
- Help promote seasonal programs amongst clients and perform seasonal intakes.
- Help promote seasonal programs amongst agencies and partners.
- Attend and participate in regular staff meetings / training and individual supervision meetings as required.
- Uphold the standards and principles of The Salvation Army’s mission and ministry.
- Other relevant duties as assigned by supervisors.
Qualifications
Bachelor’s’s Degree Required in Social Work, Psychology, Counseling, or Human Services related field from an accredited institution.
Master’s Degree in Social Work preferred.
- At least 2 years of experience in case management or counseling.
- Strong written and verbal communication skills
- Bilingual (English / Spanish) required.
- Possess organizational skills and professional demeanor.
- Must be proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook, Forms, Teams)
- Knowledgeable of Mercer County and community resources.
- Strong interpersonal skills and ability to work as part of a team.
- Excellent written and verbal communication skills, and ability to establish rapport.
- Ability to maintain the confidentiality of interactions with clients and fellow employees.
- Awareness and sensitivity to cultural and socioeconomic characteristics of populations served.
- Good organization skills and timely follow-up skills to ensure that any concerns are reported timely, clearly, and addressed in an appropriate time frame for overall cleanliness and safety.
