Job Summary :
As the Director of Case Management, you'll oversee department activities, ensure compliance, and coordinate patient care processes for optimal outcomes.
Responsibilities :
- Direct and evaluate departmental operations to achieve performance objectives.
- Establish working relationships with key stakeholders for effective care coordination.
- Assess department performance and implement improvement activities.
- Ensure compliance with policies, SOPs, and care coordination processes.
Qualifications :
- Bachelor's degree required; Master's degree preferred.
- RN licensure or LCSW required.
- 5-7 years' experience in acute care Hospital Case Management.
- 2+ years' experience in case management leadership preferred.
