Join a fast growing team of individuals who care about enhancing the lives of Seniors in our community! We are looking for a Client Care Coordinator who would be able to support Clients and their families through this aging season.
Must be CNA / HHA and sales experience preferred. You would be dealing directly with clients performing the following duties (not inclusive list) :
- Meet with potential clients and family members to discuss their needs and provide care solutions.
- Work with other team members to coordinate various aspects of a client’s care.
- Recognize and pursue opportunities to modify care plans to best support the ongoing needs of clients.
- Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
- Fill in for client shifts as needed.
Qualifications desired :
- Outgoing personality who is a high level team player
- Highly organized and able to handle consistent, necessary change
- Ability to write and speak professionally
- Desire to build trusting relationships with our clients and families
- Have some experience with duties listed above
- Be able to lift 25 pounds
- Ability to speak calmly in stressful situations
- Complete and pass a Level 2 nationwide criminal background check, DMV check and drug screen
- Possess a valid driver’s license and auto insurance with you listed as a covered driver
