Responsibilities
Position Summary :
Under the direction of the Market Manager Palliative Care IP program is responsible for providing specialized Social Work services to advocate for patients and their families of all age groups upon request of the Palliative Care team patient physician a relative and / or other concerned persons.
The Social Worker is an integral member of the Palliative Care team. The Social Worker provides services to inpatients in the hospital setting and the community.
Qualifications
Minimum :
- Palliative Care and / or Hospice experience for 1 year required.
- Master’s Degree - in Social Work.
- LIC LSW Licensed Social Worker in State of NV.
- CT CPRBLS AHA Basic Life Support - Nat'l
- Ability to work independently read and analyze patient data communicate effectively verbally and in writing.
- Responsible for highly confidential information.
- Must have a working knowledge of social work laws rules and regulations and community agencies.
Preferred Qualifications :
- Two-Three years of hospital experience as a social worker preferred.
