A Youth Case Manager enhances the quality of Peers’ management and maximizes satisfaction, coordination, provision, and monitoring of services tailored to best address Peers' strengths and needs.

Extensive case management requires frequent, planned contact with the family to assess progress toward goals to self-independency.

Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the Peer’s health and human services needed.

A Part- time position.

A case manager must be and show empathetic, caring, organized, and has strong relationship-building skills. Case managers understand and know about the different resources available in the community.

They also have excellent documentation, time management, and multitasking skills. The Case Managers are required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call.

Responsibilities :

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and Peer-centered.
  • Providing intensive and individualized case management services for program participants with a focus on empowering individuals to overcome barriers.
  • Meeting with each participant every week to provide case management supporting the successful transition into and retention in employment, education, or further training upon completion of program, and continuing to meet with graduates six months after graduating from the program.
  • Arrange abeyances with social services, health, medical, and governmental agencies.
  • Take the extra mile and interact with Peers to keep track of their progress and to ensure satisfaction.
  • Record cases information, complete accurately all necessary forms and produce statistical reports.
  • Adhere to professional standards as outlined by protocols, rules, and regulations.
  • Have a car, valid driver’s license, and insurance.
  • Be able to work some flexible hours (e.g., evenings and occasional weekends)

Life Skills Tasks :

  • Create innovative life sills curricula for use in house meetings, group projects and individual meetings, to support Peers s in increasing self-sufficiency.
  • Teach life skills to Peers on an individual and group basis, including emotion regulation, positive coping skills, career planning, interpersonal communication, daily living, household planning and upkeep, housing and money management, self-care, social relationships, employment, study skills, etc.
  • Provide resources and referrals to Peers for life skills needs, such as interview clothing, budgeting, education, transportation, legal services, etc.

Landlord Related Tasks :

  • Collect income verification, rent and other fees monthly from Peers and keep accurate and thorough written records of all.
  • Maintain proper documentation of all unit inspections and the feedback provided to Peers.
  • Build a reputable relationship with the Landlords.
  • Oversee safety and cleanliness and model how to perform basic household maintenance.
  • Coach Peers on how to complete accurate maintenance requests utilizing the facilities ticket system.

Requirements and skills :

  • Proven working experience in case management.
  • Maintains professional boundaries while treating individuals with respect and compassion.
  • Remains calm under pressure; handles emotionally challenging situations professionally and intentionally.
  • Has a passion and a calling for working one-on-one with adults who need additional support to be successful in their careers
  • Certified Peer Support
  • Excellent knowledge of case management principles, healthcare management.
  • Previous experience with psychological aspects of care
  • Effective communication skills; emotional intelligence
  • Excellent organizational and time management skills
  • Review Peers progress towards goals and objectives.
  • Maintain records and files, and prepare clear, concise, and timely reports.
  • Identify, network, and establish contact with collateral services needed by the Peer
  • Problem-solving skills and ability to multi-task.
  • Compassionate with teamwork skills.
  • Patience and active listening.

Education & Experience Requirements :

  • Some college, high communication skills, social work, or related field.
  • Demonstrated competencies in the following areas : creative problem-solving skills, strong teaching skills, leadership skills, visionary ability, opportunistic nature, and familiarity with relevant compliance rules and regulations.

Knowledge, Skills, and Abilities :

  • Ability to use a computer; expertise in Word and Excel; familiarity with other Microsoft Office products.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and organizational skills.
  • Trauma-Informed.
  • Motivational interview.

Experience :

Working with at-risk / vulnerable youth : 1 year (Required).
