Description
Nuvance Health has a network of convenient hospital and outpatient locations Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices.
Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.
Summary :
Performs all functions required to insure proper storage, preparation, dispensing, use and administration of all drugs according to accepted standards of medical, nursing and pharmacy practice and applicable hospital policy and / or law.
Responsibilities :
1. Prescription processing and dispensing Criteria A : Prescription review and processing
- Reviews prescription for completeness and appropriateness.
- Conducts patient profile reviews prior to dispensing.
- Detects and resolves order entry and prescribing problems.
- Prospectively intervenes Prescription processing and dispensing Criteria B : Drug preparation and dispensing
- Provides properly and accurately labeled medications.
- Demonstrates understanding of drug preparation techniques.
- Executes dispensing of medications in a timely manner. Insures product safety. Prescription processing and dispensing Criteria C : Documentation
- Complies with requirements for Pharmacy Department documentation in all areas in a clear and concise. Prescription processing and dispensing Criteria D : Purchasing and inventory control
- Know sources of drug acquisition
- Anticipates shortages
- Obtains drugs in select emergency situations
2. Provision of clinical and drug information Criteria A : Problem solving / decision analysis
Identifies the pertinent question, processes data to formulate solutions

- Retrieves information from available resources
- Provides general verbal and / or written drug information in accurate and timely manner.
- Understands organization of medical record
- Selects appropriate drug reference for each particular question
- Evaluates patient-specific drug information request, retrieves pertinent drug information, interprets information, and formulates an appropriate response.

Demonstrates understanding of basic pharmacotherapy of disease states.
3. Teaching, communication Criteria A : Communication

- Counsels all patients and or health care providers about their drug therapy to ensure safe medication use necessary to optimize patient outcomes
- Identifies, prevents, and resolves drug-related problems through patient education. Teaching, communication Criteria C : Teaching
- Demonstrates understanding of basic principles of teaching;
- Trains undergraduate pharmacy students or new pharmacists basic job functions.

Other information :
Required : Bachelor of Science degree in Pharmacy from an approved college of Pharmacy.
Greater than 3 years pharmacy practice experience, or 1 year of retail Pharmacy experience
Minimum Experience : one year
Desired : Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut. Pharmacist must obtain a registered license within 3 months of employment..
Member of the local, regional, and national professional organizations.
Participates in no less than 15 credit hours annually of professional pharmacy education approved by the American Council on Pharmaceutical Education.
Meets all rules and regulations of the department including requirements or competency's for age-specific or position-specific patient care standards.
Participates in departmental and interdepartmental educational, drug use evaluation, quality assurance programs and other projects as required.
