Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role : With minimal supervision, as part of the CTI (Critical Time Intervention) team and the agency leadership team, and in keeping with Agency policies and procedures and professional standards and applicable regulations, use own experience and judgment to carry out the mission of the Critical Time Intervention model (CTI) and Transitional Residential Services (TRS) for youth boarding in hospitals and ERs.
Responsibility includes monitoring the provision of CTI and TRS services for assigned youth, developing relationships with hospitals and service providers, following the fidelity of the CTI program, supervising the CTI team, TRS manager, and clinical staff, monitoring budgets, and completing required documentation, outcome data, and reports as required.
Key Responsibilities Include :
- Monitor the provision of CTI (Critical Time Intervention) and TRS (Transitional Residential Services)
- Ensure CTI staff meet with youth and family referred from hospitals for CTI services.
- Assist the CTI team and the youth / family in determining additional services needed to achieve and maintain stability.
- Review the schedule of visits with the youth / family based on CTI requirements and the youth / family’s needs.
- Monitor the provision of services at the TRS.
- Ensure that the CTI team follows the CTI process and maintains the fidelity of CTI requirements.
- Ensure CTI and TRS teams work together to meet the needs of the youth / family.
- Manage Program Expenses in Keeping with Budget Guidelines
- Oversee and monitor all budgets, financial reports, and expenses for the certified residential sites assigned.
- Review monthly financial printouts and work with the Associate Director and Director of Finance to modify the budget and / or expenses as needed.
- Ensure that the youth’s rights are protected with regard to their money and personal possessions, as applicable.
- Maintain Appropriate Records and Prepare Necessary Reports
- Ensure that all required documentation is completed with required elements and maintained per Agency procedures, including progress notes, safety plans, intake / discharge plans, and assessments.
- Ensure incident reports are completed in a timely manner and safeguards and recommendations are implemented.
- Complete youth / family satisfaction surveys and analyze results.
- Implement systems to measure quality initiatives and program outcomes. Monitor the status of program outcomes data and ensure data is submitted in a timely manner for the Agency dashboard.
- Supervision of Assigned Staff
- Supervise the CTI team, TRS Manager, and TRS Clinical staff.
- Develop and ensure the implementation of an orientation and ongoing training program for the CTI team.
- Assist the TRS Manager in developing and implementing an orientation and ongoing training program for the TRS team.
- Provide ongoing direction, training, support, and performance evaluations for assigned staff.
Education & Experience :
NYS LCSW or LMHC required. One year experience working with youth with a mental health diagnosis and / or I / DD dual diagnosis or case management experience working with youth and families preferred.
Conditions of Employment :
Candidates must have a valid NYS Driver's License in accordance with the Agency Vehicle and Driving policy and have transportation necessary to meet program needs.
The role requires on-call availability for bona fide emergencies and after-hours program service needs. Schedule flexibility is required, including occasional early morning, evening, or weekend meetings / functions.
Candidates must be willing to meet with youth / family in hospital, home, and community settings. Successful participation in required database checks through the State Central Register of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General, and Specially Designated Nationals is mandatory.
