POSITION SUMMARY

The Community Health Worker (CHW) plays a critical role in promoting health and wellness within the community by providing outreach, education, and support services to individuals and families.

The CHW works closely with healthcare providers, social services, and community organizations to bridge the gap between the community and healthcare systems, ensuring that individuals receive the care and resources they need to achieve optimal health outcomes.

Essential Functions Statement(s)

  • Conduct outreach activities to identify and engage individuals and families in need of health services.
  • Build and maintain relationships with community members, healthcare providers, and local organizations.
  • Participate in community events, health fairs, and other outreach activities to promote health education and services.
  • Provide culturally appropriate health education to individuals and groups on various health topics, including disease prevention, nutrition, mental health, and chronic disease management.
  • Develop and distribute educational materials and resources to the community.
  • Advocate for clients' needs within the healthcare system and connect them with necessary social services and resources.
  • Empower clients to take an active role in managing their health and well-being.
  • Collect and record documentation of services provided, and outcomes achieved.
  • Collect and report data on community health needs and program effectiveness.
  • Work collaboratively with healthcare providers, social workers, and other team members to coordinate care and services for clients.
  • Participate in team meetings, training sessions, and professional development opportunities.
  • All other duties as assigned.

SKILLS & ABILITIES

Education : High school diploma required. Bachelor's degree or higher in health education, social work, or a related field preferred.

Experience : Minimum of 2 years of experience as a Community Health Worker or similar role.

Computer Skills : Able to learn and utilize electronic data systems and electronic communications. Proficient in Microsoft Office.

Certificates & Licenses : Certified Community Health Worker (CHW) preferred. Valid driver's license and reliable transportation.

Other Requirements : Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse populations.

Knowledge of local healthcare and social service systems. Ability to work independently and as a part of a team. Excellent organizational and time management skills.
