SUMMARY
The Facility Outreach Coordinator collaborates with facility representatives who support patients ability to access to the non-emergency medical transportation (NEMT) program.
The requires collaboration with nursing homes, dialysis clinics, and other healthcare providers to enhance scheduling coordination and facilitate information sharing for patients with frequent transportation needs.
Key responsibilities include delivering training to facilities on utilizing NEMT services and fostering strong relationships.
DUTIES & RESPONSIBILITIES
- Serve as the lead contact for facility representatives supporting patients accessing non-emergency medical transportation (NEMT).
- Promote the organization's goals and initiatives within the community.
- Foster ongoing relationships with facilities through in-person meetings, phone calls, and group presentations.
- Conduct in-service training for facilities on utilizing NEMT services and building relationships.
- Ensure outreach and training activities are carried out in the assigned area.
- Coordinate community outreach, event planning, and educational initiatives.
- Develop and implement effective outreach strategies.
- Analyze facility service requests to provide education and outreach that resolves issues and enhances quality.
- Review patient attendance data to ensure member experience is aligned with expectations.
- Lead efforts in facility networking, tracking, and reporting outreach activities.
- Gather and analyze feedback from the community and stakeholders.
- Participate in additional projects or duties as assigned.
- Willingness to travel for business up to 80% of the time is required.
REQUIREMENTS & QUALIFICATIONS Education :
- High school diploma or G.E.D. equivalent, required
- Three years experience in roles supporting Medicaid / Medicare healthcare services in either; customer service, transportation services, utilization / case management, or social work.
- Non Emergency Medical transportation (NEMT) experience preferred, but not required.
- Clinical, case management, and discharge experience preferred.
- Or equivalent combination of education and / or experience.
