About Temecula Valley Hospital

Case Manager The Case Manager is responsible for performing assessments appropriate to functional status, diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner.

The candidate will also identify and prioritize the need for further assessment based on the patient’s clinical presentation / diagnosis, care setting, desire for care and responses to any previous care.

The Case Manager initiates discharge planning for each patient upon admission as appropriate. In addition, the Case Manager evaluates and documents the patient’s response to the Plan of Care and to interventions;

revises and updates the Plan of Care as evidenced by documentation.

Qualifications

Two (2) years’ experience in an acute care hospital of Utilization Review environmentUtilization Review experience preferredCompletion of an accredited Registered Nursing programCurrent California Registered Nurse licenseCCM and / or CPUR certification preferredCPI Certification within the first 90 days
