Location : Stanton, CA / This position requires travel between 3 Jamboree communities. These communities are located along Hwy.
39, (aka Beach Boulevard).
Status : Full-Time, Hourly

Primary Objective of Position :
The Community Rehabilitation Specialist works under the supervision of the Clinical Services Director to support residents and team members within the Community Impact department.
The Community Rehabilitation Specialist’s primary focus is to provide support and guidance to individuals with physical, mental and emotional disabilities.
The role focuses on helping residents and clients integrate into their communities, enhancing their quality of life, and promoting independence.
The Community Rehabilitation Specialist will work closely with clients, families, healthcare providers, and other community organizations to develop and implement personalized goal plans.
Major Areas of Accountability :
- Conduct comprehensive assessment to identify resident’s needs, strengths, and goals.
- Develop individualized rehabilitation plans that include strategies for improving daily living skills, vocational training, social integration and mental health support.
- Provide direct support and coaching to clients in areas such as personal care, social, communication and personal skills and basic activities of daily living (ADL).
- Work in collaboration with onsite care team, that may include coordinators, LVN and other staff specialists.
- Work in collaboration with resident’s families, healthcare providers, and community organizations to create a supportive network.
- Advocate for residents’ access to necessary services, ensuring they are treated with dignity and respect.
- Prepare and facilitate group activities and community programs that promote social inclusion and community engagement.
- Maintain accurate and up to date records of client’s progress, services provided, any changes made in their condition.
- Ensure timely and accurate input of all required data in appropriate database systems.
- Create and maintain all required documentation in resident hard file throughout the duration of program enrollment.
- Coordinate with Program Analyst to prepare regular reports on the status of enrollments, clients and program outcomes.
- Ensure compliance standards are met with all organizational and programming requirements, polices and procedures.
- Participate in ongoing training and development opportunities to enhance skills and knowledge.
- Attend regular team and community meetings as required.
Level of Education, Experience and Skill Set :
- Excellent analytical skills and attention to detail. Ability to gather and interpret data, identify trends, patterns and insights to inform decision making.
- Excellent verbal and interpersonal communication skills. The ability to convey information clearly and professionally, both verbally and in writing.
- Ability to work effectively with a team as well as independently. Build positive working relationships with colleagues, participants and stakeholders in order to reach department goals.
- Ability to multi-task. To take action and make decisions in a proactive manner to come up with innovative and creative solutions.
- Strong working knowledge of Microsoft Office (Word, Excel, PowerPoint, OneNote and Outlook), Adobe.
- Bachelor’s degree or equivalent preferred.
- Two or more year’s experience that has equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the position.
- Valid California driver’s license and availability of an insured vehicle to travel within the California service area.
Physical Demands :
The Community Rehabilitation Specialist is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel.
The employee is required to stand, walk, reach with arms and hands, climb, balance, twist, lean, move from one location to another and to stoop, kneel, crouch or crawl.
Vision abilities required by this job include close and far vision. Ability to operate a computer keyboard, and lift files and reports.
Exposure to glare from a computer.

Work Environment :
The work environment will usually be in a professional office setting, with possible shared workspace. The noise level is usually quiet to moderate.
When traveling the environment will change dependent upon the site visiting.
