Home Physical Therapist
Overview
HarmonyCares is seeking a Home Physical Therapist to provide physical therapy services in the home setting. This position involves assessing patients, setting goals, and training non-professional home health aide staff.
In addition, the candidate will need to update care plans, complete assessments, and participate in staff meetings and educational in-services.
Candidates should possess a current State Physical Therapy License, a good driving record, and at least one year of experience as a home care professional Physical Therapist.
Proficiency in clinical skills, Microsoft Office, and the ability to drive for 2-4 hours per day are also required.
- Provide Physical Therapist services including assessment, evaluation, and goal setting
- Update care plans and complete assessments
- Attend staff meetings and educational in-services
- Possess a current State Physical Therapy License and good driving record
- Have at least one year of experience as a home care professional Physical Therapist
- Proficiency in clinical skills and Microsoft Office
- Ability to drive for 2-4 hours per day
