Our Hospice Licensed Vocational Nurses enjoy some notable benefits :

• Schedule : Monday-Friday 8 : 00 AM-5 : 00 PM with possible call expectations on the weekends and holidays or

evening shift 6 days on 6 days off

As a Hospice Health Licensed Vocational Nurse, here’s a big-picture view of what you’ll do :

• Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform nursing services which are planned, delegated, and supervised by the registered nurse
• Assess and provide patient and family / caregiver education and information pertinent to diagnosis and plan of care
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

A few must-haves for Home Health Licensed Vocational Nurses :

• Graduate of an accredited vocational nursing program and active LVN license in California
• One year of hospice LVN experience
• CPR certification
• Valid California driver's license, auto insurance, and reliable transportation
• Knowledge of state and federal home health and hospice regulations
• Clinical proficiency in nursing, strong communication skills and compassion
