WelbeHealth PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities.

At the direction of the Launch Medical Director, the Travel Nurse Provider works collaboratively with WelbeHealth Physicians and the IDT, providing assessments, planning and primary care, and continuing medical education to WelbeHealth participants.

Essential Job Duties :

• Perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participants
• Collaborate with physicians and IDT to develop care plans for each participant
• Collaborate with physicians and IDT in discharge planning from acute or long-term settings
• Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
• Work collaboratively with WelbeHealth physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination
• Ability to travel to our various centers depending on the need
• At the direction of the Launch Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
• Take part in rotating night and weekend call

Job Requirements :

• Graduation from an Accredited School of Nursing
• Certificate as a Nurse Practitioner or Physician Assistant with Prescriptive Authority
• RN State License
• Active Drug Enforcement Administration (DEA)
• Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
• Minimum of two years of experience working with the senior populationCurrent knowledge of chronic care / geriatric issues and best practices
• Reliable means of transportation

Salary / Wage base range for this role is $145,000 - $174,478 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits.

Actual pay will be adjusted based on experience and other qualifications.
