Registered Nurse - Phone Triage

Job Summary :

The Registered Nurse in Phone Triage will provide professional assessments and prioritize treatments based on urgency. Utilizing critical thinking skills, this role requires independence in applying nursing knowledge to manage urgent issues in collaboration with various healthcare professionals, ensuring that patient needs and goals are met effectively.

Key Responsibilities :

• Uphold and support the mission, core values, objectives, and policies of SCPP PACE.
• Respond promptly to each incoming call.
• Perform patient assessments to determine the appropriate level of care required :
• Emergent
• Urgent
• Non-urgent
• Home care status
• Provide home care instructions using approved guidelines and reference materials when appropriate.
• Communicate and collaborate on urgent issues with paramedics, pharmacy, administration, home care coordinators, and primary care providers effectively.
• Initiate emergency treatment as necessary.
• Facilitate hospital transitions by faxing current MAR, DPOA, and Code status paperwork to medical facilities.
• Document thoroughly and accurately using appropriate computer software, adhering to approved policies and procedures.
• Actively participate in new employee orientation, in-service programs, staff meetings, continuous quality improvement initiatives, and periodic performance evaluations.

Required Qualifications :

• Graduate of an accredited School of Nursing.
• Current Nurse licensure in the State of Michigan (LPN or RN).
• Minimum of one year of experience working with the frail elderly or long-term care population.
• Minimum of one year of home care experience.
• At least two years of nursing experience.
• Participation in continuing education programs.
• Familiarity with participant interactions via telephone.
• Strong ability to establish and maintain effective working relationships with participants, coworkers, and medical service providers.
• Effective communication and interpersonal skills.

Preferred Qualifications :

Experience in a high-stress healthcare environment.

Certifications :

Maintain current nursing licensure by completing renewal applications and meeting continuing education requirements.

Specific Requirements :

• Possess time-sensitive clinical decision-making skills and critical thinking competencies to accurately identify participant problems.
• Provide exceptional customer service to staff, external partners, and participants.
• Make independent nursing decisions as needed.
• Communicate effectively both verbally and in writing; express thoughts logically and clearly.
• Ability to work well under stress and relate to the public, nurses, and physicians tactfully.
• Adequate typing skills and basic computer knowledge.
• Maintain a conducive work environment to be fully present during shifts, ensuring adequate cellular and internet service.
• Access to a computer terminal and telephonic equipment for remote / home office use.
