Key ResponsibilitiesConduct virtual consultations focusing on patient education related to chronic diseasesReview and respond to remotely transmitted patient data such as blood pressure and blood glucoseEstablish relationships with patients to empower them in managing their healthRequired QualificationsCurrent, valid, and in good standing Alaska Nursing License (LPN / LVN)3+ years of experience in cardiology, primary care, home care, or chronic care managementExperience in remote monitoring and / or virtual care management programs (preferred)HIPAA-compliant area to work remotely and a distraction-free work environmentReliable internet connection with minimum 100mbps download and 10mbps upload speed
