The Care Coordinator provides complex and diversified support to the Care Team (clinician, medical assistant, nurse, pharmacy, etc.

by coordinating care for patients with serious, complex, or chronic health problems, or those with psychosocial issues.

Ideal candidates will maintain and enhance the health and well-being of patients by participating in the team effort of the department by facilitating patient flow, assisting with procedures, telephone communication, and follow-up diagnostic procedures within the scope of a WA State Medical Assistant-Certified .

Schedule is Monday through Friday from 8 : 00am to 5 : 00pm. ESSENTIAL DUTIES AND RESPONSIBILITIES : In-office patient care responsibilities including but not limited to :

  • Rooming patients, taking patient vitals, and documenting accurately
  • Assisting with patient procedures within scope of practice
  • Administering ordered immunizations and medications
  • Initiating tests and / or procedures as ordered.
  • Prepare equipment, paperwork and supplies in room for clinician exam.
  • Assist clinician by chaperoning, as appropriate, and assisting with exams and minor procedures. Serve as an effective communication link between patient and clinic staff by gathering / sharing information to / from patients including but not limited to :
  • Give patients educational material, instructions, and after visit summaries (AVS).
  • Gather and communicate laboratory results to patients as directed by clinician.
  • Working the EMR in-basket for prescription refills, MyChart communications, prior authorizations and telephone encounters within scope of practice.
  • Contact patients for check-ins as directed by clinician to determine if scheduling is indicated.
  • Complete advanced care planning documents and witness signing as directed.
  • Answer incoming department calls, document reason for call, and direct to appropriate team member.
  • Independently and proactively identify patients in the practice by running panel reports for specific diagnosis and preventive care needs.

Maintaining the patient panel documents including notification of ER and Inpatient admissions.

  • Monitors new referral work queue and outreaches patients to schedule within 24-48 business hours.
  • Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
  • Evaluate and analyze patient records based on primary Care patient standards for patients who need : preventive services, diagnostics, and follow up.
  • Schedule patients for TCC / Pall Med appointments and assists in the scheduling of specialist appointments at the direction of RN Care Manager and Clinicians, including assisting patients with transportation scheduling and resources.
  • Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients. SKILLS AND ABILITIES :
  • Able to consistently demonstrate essential clinical competencies for this role and license / certification.
  • Ability to maintain patient privacy and confidentiality.
  • Ability to collaborate effectively and proactively with other multi-disciplinary team members.
  • Ability to efficiently manage time and prioritize multiple tasks.
  • Strong interpersonal and communication skills.
  • Must be willing and able to float between departments as needed.
  • Knowledge of medical terminology.
  • Philosophy and values consistent with a patient-centered care model.
  • Good organizational skills.
  • Demonstrated problem solving skills in a complex environment.
  • Proficiency working in an electronic medical record system and Microsoft Outlook, Work, and Excel. EDUCATION AND EXPERIENCE :
  • WA State Medical Assistant-Certified.
  • Current Healthcare provider CPR certification, American Heart Association BLS.
  • At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.

Pay Range : $24.21 - $33.90 The above information is intended to indicate the general nature and level of work required in this position.

Benefits-eligible employees accrue PTO and Personal Time based on hours worked and State worked, totaling 120 hours in the first year for full time staff and 200 hours in the first year for full time supervisors and above, increasing in subsequent years.

PTO and Personal Time accruals are pro-rated by FTE / hours worked. Non-benefits eligible employees will accrue Personal Time based on hours worked and State worked.
