Overview :

The Community Health Outreach Worker is a front-line public health worker who is a trusted member of and / or has a close understanding of the community served.

This trusting relationship enables the worker to serve as a liaison / link between health / social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

Qualifications :

  • Education : High School Diploma or GED is required.
  • Experience :
  • 2 years of experience as a Community Health Worker or similar role is required.
  • Ability to write and speak English and Spanish is required.
  • Certification : Community Health Worker certification through Department of State Health Services in the state of practice is required.
  • Other : Valid driver's license in the state of practice is required.
  • Other Skills / Knowledge : Strong relationship and collaboration skills, excellent communication skills, ability to work independently, basic knowledge of Microsoft Office, emails, and word documents.
