The Position : We are looking for a full-time Nurse Practitioner in Lowell, MA to provide care to patients in both a clinic and home / facility setting.

- $111k- $145k Salary, Based on Experience

Job Responsibilities
- Monday to Friday (8am to 4pm)
- Performs Initial Assessments for all New Participants and Conducts Quarterly Evaluations During their First Year of Enrollment.
- Assesses Participants for Episodic Appointments, Acute Illnesses, and Follow-up Care After Hospital Stays.
- Visits Nursing Homes for Acute or Chronic Situations when Participants are Unable to get to the Clinic
- Conducts Home Visits for Acute or Chronic Situations when Participants are Homebound.
- 10-15 Patients a Day
- Hands on Care for Geriatric Patients
- Participates in On Call, which is By Phone Only
Job Requirements
- MA Nurse Practitioner License Required
- 1+Years of Experience as a Nurse Practitioner Required
- MA DEA License Required
- Current CPR Certification
- COVID Vaccination Required OR Exemption
