What you will Do : * Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.
- Provides health coaching / education to patients / families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER / Urgent usage.
- Delivers nursing care for the clients in their homes, utilizing appropriate available resources.* Involves the client / caregiver in developing and carrying out the plan of care.
Minimum Qualifications : Education : * Current state Registered Nurse License or permit from State Board of Nursing required.
- Completion of ADN required, BSN preferred. Experience : Must have demonstrated competency in an acute patient care setting for at least one year in medical, surgical or intensive care setting.
