Employment Type : Full time Shift : Day Shift Description : Position Purpose : Provides nursing services to patients and families at end of life that include assessment, planning, intervention and evaluation in the home setting or nursing home.
What you will Do : + Provides competent ongoing holistic nursing assessments, planning, palliative interventions and evaluations, individualized patient and family centered care utilizing knowledge of diverse lifestyles, family structures, and dynamic cultural and religious practices, learning preferences, and age and gender specific considerations.
- Collaborates in the development, review and revision of interdisciplinary team plan of care.+ Oversees Hospice Aide services.
- Utilizes Hospice resources efficiently.+ Demonstrates flexibility to meet patient / family needs across Hospice of North Iowa.
- Able to maintain accepted patient / nurse caseload within scheduled hours.+ Demonstrates understanding of rules and regulations governing hospice services.
- Maintains accurate clinical record in timely manner and demonstrates competency with EMR and tablets.+ Participates in regularly scheduled meetings, in-services and quality assurance / improvement activities.
Participates in QAPI program.+ Demonstrates autonomy, professional judgment, responsibility, reliability, service excellence and in individual development.
Hours / Schedule : Full Time Monday-Friday Days with week night and weekday call rotation.
