Responsible for the overall management of all social work services for assigned home care clients. Under the direction of the client’s primary care physician and Hospice Medical Director, plans and provides individualized social work intervention to enable clients to maintain dignity, independence and choice during the end stage of life.
Participates as a member of the IDT to plan and provide holistic care to patients and families. Interventions evolve from ongoing psychosocial assessment and are in accordance with the IDT Care Plan.
Desired Qualification :
- Master’s or Bachelor’s Degree in Social Work and one year professional social work experience in a healthcare environment.
- Current Basic Life Saving for Health Care Providers (CPR and AED) certification.
- A valid driver’s license, acceptable driving record and personal vehicle for on-the-job transportation with evidence of auto insurance coverage as required by the State of Washington.
