The Community Health Worker is responsible for assessing, planning, and implementing individualized care strategies for patients.
They work as a liaison with Medicaid members to ensure access to appropriate care and provide home and social assessments.
This position requires local travel within specified communities in Missouri.
Key Responsibilities :
- Assess, plan, and implement individualized care strategies
- Utilize company and community resources to establish effective case management plans
- Collaborate with patients, families, and healthcare providers
- Identify and initiate referrals for social service programs
- Coordinate with care team and advocate for patients
- Document member assessments, care plans, and referrals provided
- Proactively engage members to manage their own health and healthcare
- Provide member education on community resources and benefits
Requirements :
- 4+ years of case management, community outreach work, or volunteering experience
- Ability to travel locally up to 50% of the time
- Access to a designated quiet workspace in your home
- Valid Missouri driver's license and current automobile insurance
Preferred Qualifications :
- Bachelor's Degree in a health-related field
- Community Health Worker (CHW) Accreditation
- Care management experience
- Knowledge of Medicaid / Medicare population
