Job Summary
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Job Responsibilities
- Interviews patients, assesses priorities, and writes reports while managing complex caseload.
- Completes comprehensive assessments of patients.
- Assists patients / families with referrals to community agencies and provides education and resource information.
- Works in collaboration with the interdisciplinary team to ensure quality support of care and safety of discharge.
- Collaborates with leadership on complex discharge planning issues and delays in service.
- Provides timely and concise documentation in the medical record, the assessment, concerns, needs, and discharge plan.
- Keeps plans up to date as patient progresses toward discharge.
- Supervises and / or trains other new employees, students, or interns as needed.
Qualifications
- Associate's degree or higher in Social Work required upon hire
- No experience required
- LSW - Licensed Social Worker in the State of Kentucky required upon hire OR
- LCSW - Licensed Clinical Social Worker in the State of Kentucky required upon hire OR
- CSW - Certified Social Worker in the State of Kentucky required upon hire
