JobSummary :
Serves as anintegral member of emergency department / CDU care team. Providespsychosocial assessments coordination and implementation of ageappropriate social medicine services to meet the needs ofpatients / families / significant others in the emergency departmentandCDU.
EssentialResponsibilities :
Participatesas a member of the ED / CDU care team providing social medicineexpertise in response to care team referrals and case finding ofhigh need populations (e.
g. new diagnosis terminal illness loss offunction lack of social support grief and bereavement homelesscomplex finances etc.)
Completespsychosocial assessments of patients in ED / CDU. Identifiespsychosocial needs develops and implements plan of care.
Documentsaccording to departmental standards Federal and State regulations.Assures medical record documentation of patient interaction andrelated tasks performed in a timelymanner.
- Upon ED / CDU referral or casefinding assists patients and their support system (e.g. friendsfamily) with coping strategies related toillness.
- Conducts social medicineconsultation and assistance for cases invloving abuse neglectadvance care planning and legal decision making (e.gguardianship)
- Provides informationand referrals to community resources for post dischargesupport.
- Applies knowledge basetheoretical concepts specific functional skills and essentialsocial values used to effect change in human behavior emotionalresponses and socialconditions.
- Contributes to thepatient discharge planning process by assessing patient and supportsystems choices and educating them on the risks and benefits oftheir choices to facilitatedischarge.
- Assists in coordinationof discharge plan and assures patients placement in approrpiatecommunity based settings fromED / CDU.
- Provides informationresource identification and referral services mediation servicesadvocacy services and education to patients and their supportsystem.
- Plans for and participatesin development activities to keep abreast of current trends inmedical social work. Participates in performance improvement.
Implements Quality Improvement processes. Assists with developmentand implementation of departments annual workplan. Performsclinical record audits.
Maintains educational record and providesevidence of competency. Participates in departmental functions suchas staff meetings staff inservices and committees as assigned.
Participates in orientation of newpersonnel.
Requirements
BasicQualifications :
Experience
Minimumone (1) year experience in health caresetting.
Education
Mastersdegree in SocialWork.
LicenseCertificationRegistration
- LicensedClinical Social Worker (Hawaii) OR Licensed Social Worker(Hawaii)
- BasicLife Support within 0.5 months of hire from American HeartAssociation
AdditionalRequirements :
- Experiencewith interdisciplinary teamfunction.
- Knowledge of medicalterminology the psychosocial impact of seriousillnesses.
- Demonstrated ability tointerrelate with physicians nurses support staff andpatients.
- Familiar with careprocesses and systems in settings internal and external to KaiserPermanente.
- Knowledge of theprinciples of patient teaching and delegation care planning andappropriate utilization of acute hospital long term care and homecare resources.
- Demonstratescustomerfocused serviceskills.
- Knowledge of communityresources family dynamics and the dyingprocess.
- Keyboarding or typingskills.

Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker(Hawaii) Basic Life Support Certification from American HeartAssociation Healthcare experience working with medical diagnoses.
Demonstrated ability in networking with communityagencies.
