Kuakini Medical Center Description of Position
Performs psychosocial assessments of patients / significantothers to identify issues related to adjustment to illness, crisis interventionand discharge planning.
Provides directservices of brief psychosocial counseling, discharge planning, education, andserves as community resources linkage to assist patients in achieving theiroptimal level of functioning, and to ensure the continuity of care.
Engages in teaching, performance improvement,and other activities as assigned.
Qualifications and Requirements
Master’sDegree in Social Work from an accredited School of Social Work required. Requires a current State of Hawaii SocialWork License and two (2) years of independent practice as a Licensed SocialWorker.
Preferred work experienceincludes acute hospitals, long term care facilities and geriatric careservices. Previous contact andinteraction with community resources and government social service programs ispreferred;
or any combination of education and experience which would providethe necessary knowledge and abilities to meet the minimum qualifications toperform the essential functions of this position.
Starting Salary $79,934.00
