PrEP Retention Coordinator Description

Philadelphia FIGHT Community Health Center's HIV Pre-Exposure Prophylaxis Program is a nationally recognized program in PrEP delivery and implementation, as well as patient care.

The PrEP Retention Coordinator is a full-time position located at the Youth Health Empowerment Project (Y-HEP) Health Center to support the clinical team in PrEP care.

This position works collaboratively with the PrEP team at John Bell Health Center (JBHC) to coordinate PrEP care services across FIGHT's community health centers.

This position will be the primary on-site staff supporting the day-to-day implementation of Y-HEP's PrEP & primary care initiative.

The Pre-Exposure Prophylaxis (PrEP) Retention Coordinator plays a key role in the implementation of the PrEP program including but not limited to : health education, linkage to care, re-engagement and retention of PrEP patients, patient navigation, and PrEP adherence counseling.

Qualifications : High School Diploma required, Bachelor's degree is preferred. 1-2 years of experience conducting non-medical assessments and client centered education is required.

Strong computer literacy with proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint) required. PA HIV Testing Certification is preferred.

Previous experience using an Electronic Medical Record System (EMR) is preferred. Other qualifications include :

  • Knowledge of PrEP and combination HIV prevention and treatment
  • Knowledge in science and medicine
  • Proven track record of organization, attention to detail and ability to respond promptly to requests, anticipate agency needs, and maintain efficient clinical operations
  • Ability to work across teams effectively
  • Sensitive to cultural, economic, gender, and sexual orientation differences

Responsibilities include :

  • Conduct health screenings and risk assessments of individuals who are interested in Pre-Exposure Prophylaxis. Provide client-centered health education on topics such as HIV & STI prevention, medication adherence, and harm reduction.
  • Provide ongoing education about when and how to stop PrEP if desired, when and how to reinitiate PrEP in the future, when to call a healthcare provider with questions, and when & how to schedule an appointment with the Y-HEP Health Center.
  • Facilitate linkage-to-PrEP care at Y-HEP Health Center in order to recruit YHEP participants, high-risk persons and other individuals at increased vulnerability to HIV in Philadelphia.
  • Field phone calls coming in to the PrEP Hotline and provide culturally sensitive, medically accurate information about HIV and Pre-Exposure Prophylaxis.
  • Support clinical staff at the Y-HEP Health Center to retain and empower patients in the PrEP program by :
  • Confirming quarterly appointments are scheduled with a healthcare provider,
  • Ensuring patients are compliant with quarterly baseline bloodwork appointments,
  • Coordinating medication refills,
  • Reapplying for medication assistance programs in a timely fashion,
  • Conducting Rapid HIV Tests as part of ongoing PrEP care,
  • Making reminder phone calls to patients with upcoming appointments,
  • Link patients into PrEP who have recently been diagnosed with an STI or have been prescribed Post-Exposure Prophylaxis
  • Responsible for documenting all patient encounters in the electronic medical records within 48 hours, including but not limited to phone conversations, counseling sessions, HIV rapid test results, and medication pick-ups.
  • Submit applications for patient assistance programs, when possible, on behalf of uninsured, underinsured patients; monitor status of PrEP applications, and resubmit prescription requests in a timely fashion.
  • Manage PrEP medication distribution for Y-HEP Health Center patients; track and support patients with daily, weekly, monthly or quarterly PrEP med pickup schedules;

pick up PrEP prescriptions from Community Walgreens, update the PrEP prescription log, maintain a supply of keychain pill holders.

  • Work with the Manager of PrEP Programs to cultivate community partnerships, coordinate outreach, and educational activities around specific health awareness activities, participate in community health fairs as needed
  • Provide outreach to all relevant communities who may benefit from PrEP.
  • Support linkage to care efforts for PrEP participants who may be newly diagnosed with HIV.
  • Promote services and programs at Philadelphia FIGHT Community Health Centers.
  • Refer patients to appropriate programs within Philadelphia FIGHT's Community Health Centers.
  • Attend scheduled meetings, case conferences, and trainings as required.
  • Demonstrate respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
  • Other duties as assigned.

Other : Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.

Salary range : Salaried position. Benefits including health benefits as per employee handbook begin on the first day of work.

Travel Requirements : Minimal local travel required between Philadelphia FIGHT Health Centers.

Work Hours : M-F 9 : 00am - 5 : 00pm, in alignment with YHEP Health Center clinic hours and may require occasional late afternoons and evenings;

may require weekend and / or late-night activities with outreach team.
