Key Responsibilities :

  • Medication Management : Oversee patient medication plans to ensure safety and efficacy.
  • Expert Consultation : Collaborate with physicians, nurses, and other healthcare professionals on medication therapies, side effects, and drug interactions.
  • Patient Education : Provide patients with critical information on proper medication use and adherence strategies.
  • Interdisciplinary Collaboration : Partner with healthcare team members to optimize treatment plans.
  • Medication Reconciliation : Conduct thorough reviews during patient admissions, transfers, and discharges to prevent errors.
  • Regulatory Compliance : Ensure adherence to all pharmacy laws, regulations, and safety standards.

Qualifications :

  • PharmD or RPh from an accredited school.
  • Active and valid license to practice.
  • 1+ years of Clinical Pharmacist experience required.
  • Strong understanding of DEA and FDA health regulations.
  • Detail-oriented with a high level of accuracy and critical thinking skills.
