OverviewLifeLong Medical Care has an amazing opportunity for a highly motivated Registered Nurse - Triage to join our multidisciplinary team providing exceptional primary care services at our Downtown Oakland Center (DOC).

The ideal candidate must be dedicated to providing high-quality primary care to patients with complex medical conditions and passionate about working collaboratively in a team model of care.

As a key member of our team, you will work under the general supervision of the Associate Medical Director and Center Manager to provide outstanding clinical care and integrate whole-person care into our new team model.

You will play a critical role in telephonic and in-person patient triage and chronic care management.We offer ongoing education and collaboration with medical and behavioral health providers to ensure optimal health outcomes for our patient populations.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

About us : As a Federally Qualified Health Center in Northern California with over 45 years of dedicated service to the community, LifeLong serves over 66,000 patients in the East Bay Area's socio-economically underserved regions.

We provide medical, dental, behavioral health, and school-based services at more than 15 primary care sites across Oakland, Berkeley, and West Contra Costa County.

Benefits : Compensation : $97,760-$96,820 year, depending on years licensed, additional 3% pay increase for candidates with a Master's degree.

Unless a reasonable medical or religious accommodation is approved, all employees must receive COVID-19 vaccinations.Responsibilities* Coordinate team based care by focusing on the intersection between medical and social needs of patients* Acts as a resource for support staff for triaging walk in and telephonic care.

Plays a role in the new patient intake process, working closely with primary care providers to ensure complete evaluation of patient needs.

  • Facilitates chronic disease management activities* Coordinates primary care clinic services with other providers and agencies.
  • Coordinates with Care Transitions team for all hospitalized patients to ensures timely follow up and return to primary care* Uses clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications.
  • Coordinates care closely with other team members to identify patient self-care goals and to assure that medical and psychosocial needs are met.

Qualifications* Ability to effectively coordinate care with medical assistants and other support staff, encourage and nurture development and growth, to build a solid and productive team.

  • Commitment to providing primary care services for the underserved with demonstrated ability and sensitivity in working with people from low-income populations with diverse educational, lifestyle, ethnic, and cultural origins.
  • Strong organizational, administrative, and problem-solving skills, flexibility, and adaptability to change.* Ability to effectively present information to others, including employees, community partners, and vendors.
  • Ability to seek direction / approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
  • Work in a team-oriented environment with several professionals with different work styles and support needs.* Excellent interpersonal, verbal, and written skills.
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident, and sensitive staff.

Job Requirements* Bachelor's or associate degree in Nursing.* Current and valid California Registered Nurse license and BLS certification.

  • Proficient in Microsoft office suite.* Knowledge of electronic health records (EHR), Epic preferred.Job Preferences* Minimum one-year experience in community health as an RN preferred, but new graduates are welcome.
  • Knowledge of outpatient care, triage and clinic flow.* Demonstrated ability and sensitivity to provide services to unhoused, undocumented, substance users, HIV (AIDS) infected, disabled, and psychologically impaired.* Bilingual - Spanish
