We are seeking a dedicated Permanent Primary Care Physician to join our esteemed healthcare team, providing comprehensive medical care to senior care patients This role involves diagnosing and treating a wide range of conditions, emphasizing preventive care and patient education to promote long-term health and wellness.

The Primary Care Physician performs in-person comprehensive assessments and coordinates with the IDT to develop a care plan for each participant.
At the direction of the Medical Director, the Primary Care Physician oversees all care of participants in nursing homes and hospitals including admits and discharges for acute, short-term, and long-term placement and the use of medical specialists to ensure effective care coordination.
The Primary Care Physician also provides direction and support to Nurse Practitioners and other members of the IDT.
Responsibilities
Provide comprehensive and continuous medical care to patients of all ages.
Diagnose and treat a wide range of acute and chronic illnesses.
Conduct thorough physical examinations and interpret diagnostic tests.
Develop and implement individualized treatment plans for patients.
Offer preventive care and health education to promote wellness.
Coordinate with specialists and other healthcare providers for optimal patient outcomes.
Maintain accurate and up-to-date patient medical records and documentation.
Qualifications
Possess a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
Hold an active and unrestricted medical license in the relevant state of practice.
Demonstrate board certification in Family Medicine, Internal Medicine, or a related primary care specialty.
Exhibit a minimum of three years of clinical experience in a primary care setting.
Maintain up-to-date knowledge of medical protocols, treatments, and best practices.
Show proficiency in electronic health record (EHR) systems and medical documentation.
Display excellent interpersonal and communication skills to effectively interact with patients and healthcare team members.
