As a member of the Comprehensive Case Management (CCM) Team, the Substance Abuse Counselor conducts screenings and assessments and develop and implement person-centered treatment plans, and supportive services for formerly homeless residents in the Safer Haven program who are seriously mentally ill adults with co-occurring substance abuse disorders and / chronic health conditions.
Services to clients will be delivered through evidence-based client treatment practices, including Individualized Dual Diagnosis Treatment (IDDT);
the Multicultural Relational Approach™; Wellness Self-Management (WSM); Motivational Interviewing (MI); and Individualized Wellness Plans (IWP).
The person in this position reports to the Program Director.Shift : Sun-Thur 9AM-5AMEssential DutiesParticipating as a member of the Comprehensive Case Management (CCM) team in developing and delivering enhanced and expanded, culturally appropriate, evidence-based Individualized Dual Diagnosis Treatment (IDDT) for formerly homeless residents who are seriously mentally ill with co-occurring substance abuse disorders and chronic health conditions to enable them to function and live independently.
Services to clients will be delivered through evidence-based client treatment practices, including Individualized Dual Diagnosis Treatment (IDDT);
the Multicultural Relational Approach™; Wellness Self-Management (WSM); Motivational Interviewing (MI); and Individualized Wellness Plans (IWP).
Assessing clients to identify interrelated issues of substance abuse, health, behavioral health, and other care needs and determine appropriate internal and external referrals;
Providing individual, family, and group counseling;Facilitating psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc.
for residents their family and / or support networks;Providing CCM services as identified for each client;Assisting clients in achieving and maintaining functional improvements in self-care, employment, or personal relationships;
Assisting clients in achieving housing stability through discharge and post-discharge;Engaging in discharge planning and linking clients with at least 90 days of post discharge services.
Maintaining client case records;Writing client documentation and program narrative and / or statistical reports as needed.
Participating in program evaluation activities as required.QualificationsEducation and Experience Requirement(s)High School Diploma or Associates degree and Credentialed Alcohol and Substance Abuse Counselor (CASAC) with five (5) years of substance abuse counseling experience;
Bachelor’s degree with CASAC-T and 2 years of experience. Experience with individuals with HIV / AIDS, chronic mental illness, and / or chemical dependence and their families a plus.
