Job Title Experienced Caregiver or STNA Job Description Job Summary :

Provides assistance with activities of daily living, including personal care, dressing, ambulation / transferring, toileting, ambulation, light housekeeping, laundry, meal preparation and exercises as detailed in the Clients Plan of Care.

If appropriate, client transportation may also be provided.

Duties and Responsibilities :

Assists with and encourage client participation with activities of daily living to include but not limited to, bathing, dressing, ambulation and exercise.

Assists client with ambulation, including lifting and transfers as appropriate.

Provides reminders for medication self-administration

Assists with light housekeeping and laundry.

Assists in meal preparation and feeding of client as necessary.

Assist in client transportation when appropriate.

Insures a safe and sanitary environment.

Respects and insures safe care of clients clothing and valuables.

Reports all changes in clients condition to nursing supervisor and / or program director.

Provides documentation that is compliant with agency, state and federal regulations.

Performs other duties as assigned by supervisor.

Utilizes Universal Precautions.

Completes 8-12 hours of in-service education annually.

Maintains active First Aid and CPR certification (every two years).

Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices.

Also contributes to the control of hazards in specified area of responsibility.

Performs other duties or special projects as required or as assigned.

Job Requirements Minimum Qualifications

Educational / Training Requirements :

High school diploma or GED.

Licenses / Certifications :

Valid Ohio drivers license with agency approved driving record and car insurance if driving clients on behalf of agency

Current if driving clients on behalf of the agency

Knowledge, Skills and Abilities :

Strong interpersonal skills

The ability to safely transfer clients

The ability to read, write and communicate in a caring, compassionate manner to the client, work independently, be a team player

Strong time management skills

Physical Demands :

The ability to lift 50 pounds
