Job Overview :
We are seeking a passionate and skilled RN Case Manager to join our growing team. As an RN Case Manager, you will play a crucial role in coordinating and providing top-quality care to our clients.
You will oversee client care plans, supervise home care services, and act as a liaison between clients, their families, and healthcare providers to ensure the highest standard of care.
Key Responsibilities :
- Assess and develop personalized care plans based on client needs.
- Administer and oversee infusions in the home, ensuring the safe and effective delivery of medication.
- Monitor and evaluate patient progress and adjust care plans as needed.
- Provide leadership, support, and guidance to home health aides and nursing staff.
- Ensure compliance with all state and federal healthcare regulations.
- Collaborate with physicians and other healthcare professionals to coordinate client care.
- Maintain accurate and timely documentation in accordance with BrightStar Care standards.
Qualifications :
- Current and valid RN license in the state of Nebraska.
- Experience in home health, hospice, or a related field preferred.
- Experience with administering infusions in the home setting is a plus.
- Strong clinical skills and knowledge of nursing practices.
- Excellent communication and organizational skills.
- Ability to work independently and manage multiple client cases.
- Compassionate, with a dedication to high-quality patient care.
