Reports to : Director Of Nursing

POSITION SUMMARY :

Provides health education promotion and counseling to the clients of the Brockton Neighborhood Health Center according to the scope of practice and the established policies and procedures of the BNHC.

Provides diabetes self-management education to patients.

PRINCIPAL DUTIES AND RESPONSIBILITIES :

Professional responsibilities

Education :

  • Provides nutritional counseling and education to clients.
  • Collaborates with the client to design a care plan based on their learning needs including individual and group settings as well as families and caregivers.

Consultation :

  • Consults and / or refers to external agencies or professional groups as appropriate.
  • Collaborates with other BNHC caregivers regarding patient education and management.
  • Integrates activities of health promotion and education within the community by establishing and maintaining professional relationships and networking.

Clinical :

  • Provides culturally and linguistically sensitive care, using interpreters as appropriate.
  • Participates in BNHC community outreach activities based on the community needs.
  • Provides support to the health center staff on health promotion and education issues relating to clinical practice.
  • Assumes responsibility for maintaining current knowledge and competency in clinical practice and health care issues

Research :

  • Assesses effectiveness of clinical practice and utilizes results to improve practice.
  • Utilizes and applies research findings relating to community health education.
  • Supports and assists the research initiatives at the BNHC.

MINIMUM SKILLS AND KNOWLEDGE REQUIREMENTS :

Registered nurse or registered dietician with certification in diabetes.

WORKING CONDITIONS :

Works in a busy community health center in an urban neighborhood. Physical demands require lifting small office supplies (not over 25 lbs.

mostly sedentary with some walking within the office and to and from other departments. Interruptions are present and can be disruptive to workflow.

Stress is present due to the high volume of work, diversified nature of activities and frequent crises and deadlines. Performs other employee duties to cover for staff absences.

Works overtime as needed for emergencies or to meet project deadlines. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.

BNHC serves an extremely diverse patient population including Brazilian, Cape Verdean, Haitian, Latino, Portuguese, African American and Caucasian, 77% of whom live in poverty.

BNHC is committed to providing a safe and heathy environment for patients and staff. As a condition of employment, employees, to include new hires, are required to be fully vaccinated to include the most recent COVID 19 booster and annual flu vaccines, with the exception of those who have documented medical or religious exemptions, that must be approved by BNHC according to its protocols.

Additional vaccines may be required based on the recommendation of the CDC. New hires who are not yet vaccinated for flu or COVID can arrange to be vaccinated on site.
