Job Responsibilities :
- Provide Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical / cognitive functioning
- Be involved in patient and caregiver education, supervision, and reporting on patient progress
- Perform Physical Therapy assessments and develop treatment programs
- Record and report patient's reaction to treatment and condition changes
- Instruct patients and caregivers in therapy programs and device use
- Supervise physical therapy assistant and, when appropriate, the certified nursing assistant
- Prepare reports and participate in staff and Agency development activities
Job Qualifications :
- Active PT state license (IL state)
- Master's degree
- 1 yr experience in healthcare setting
