Job Summary
The medication adherence representative is responsible for making outbound calls to patients and performing medication therapy management services such as medication adherence, comprehensive medication review, and other patient services.
The representative will be responsible for answering patients’ questions, appointment confirmations, and logging patient demographics in an EHR.

- Able to work 10 hours a week
- Bilingual (Not required but preferred)
Required Qualifications
- Enrolled at Western University Health Sciences College of Pharmacy
- Good academic standing

Attend and participate in trainings and meetings.
Percentage Of Time 5% Job Duty
- Answer incoming calls and conduct medication adherence
- Make outbound telephonic calls
- Confirm patient appointments 24-48 hours in advance
Percentage Of Time 20% Job Duty
- Collect patient demographics and pertinent information related to their health
- Relay clinical questions out of scope to supervising pharmacist
Percentage Of Time 10% Job Duty
- Perform medication adherence services such as reviewing patient’s medications and whether their medications are being taken appropriately and on time
- Fill out Medicare documents (Cover Letter, Medication Action Plan, and Personal Medication List)
Percentage Of Time 50% Job Duty
- Analyze individual metrics related to your performance
- Document findings and interventions
- Communicate with providers
