Mental Health Housing Admissions Specialist
Reports to : Mental Health Admissions and Benefits Coordinator
Location : 6520 Basile Rowe, East Syracuse, NY 13057
Full Time : Monday - Friday 8 : 30 a.m. - 4 : 30 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement.
Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
Main Function :
Under the supervision of the Mental Health Admissions & Benefits Coordinator, the Admissions Specialist performs support services related to participant admission and to assigned programs to maximize occupancy in the Agency’s Mental Health housing programs.
Duties :
1. Monitors and Coordinates Admission Needs
- Collaborates with the Admissions & Benefits Coordinator in receiving and processing MH Housing referrals to licensed and unlicensed programs, as assigned.
- Supports the MH Admissions & Benefits Coordinator in obtaining pre-admission documentation related to incoming referrals, as assigned.
- Completes assessment and determination of eligibility for admission to certified residential services.
- Collaborates with housing staff to establish participant funding sources and utilizes knowledge of entitlements systems (SSI, SSD, PA, Self-Pay, etc.
to resolve barriers to payment.
- Participates in intake / admission / discharge meetings with MH Housing program supervisors.
- Supports a process of consultation with appropriate parties (applicants, families, referral sources) throughout admission decision-making process.
2. Contributes to Community Assessment of Residential Needs
- Participates in regional committees to regarding local residential services as assigned.
- Supports the Admissions & Benefits Coordinator in assessing local resources to assist potential MH Housing residents in securing safe housing placements.
- Supports the Admissions & Benefits Coordinator in conducting Social Determinants of Health assessments for participants or community members as assigned.
3. Demonstrates Accurate Record Keeping
- Documents services provided in a manner consistent with agency standards & procedures.
- Completes intake assessments with program applicants related to financial information, medication administration, and quality of life, as assigned.
- Supports the Admissions & Benefits Coordinator in maintaining records related to the MH Housing intake process, including ensuring proper archival of documents as assigned.
4. Provides Community Education
- Provides information regarding community and program services to prospective program participants, their family members, and significant others.
- Provides information about Agency programs to other providers and community members.
5. Community and Teamwork
- Works collaboratively with participants, families, program staff, co-workers, and supervisor.
- Participates in agency and community meetings, committees, and task teams as assigned.
- Participates in development of training programs and implementation of training of agency staff.
- Participates in training opportunities to build skills as assigned.
6. Follows Agency standards and procedures
- Follows agency standards and procedures.
- Maintains compliance with applicable rules and regulations.
- Provides person-centered, quality services.
7. Other duties as assigned
Education / Experience :
Associate’s degree and two years of professional mental health experience preferred. Experience in human services may be substituted for education.
Assessment experience desired.
Conditions of Employment :
Flexibility will be required to fulfill the above functions. Must possess a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
