Key Responsibilities :
- Work alongside the clinical team in assigned facilities; coordinating care plans to provide top quality care
- Track admission / review dayes on managed care plans to ensure timely submission of clinical documents
- Review care plans to ensure therapy and clinical goals are set properly
- Participate in staff meetings, regular trainings, and other collaborative mettings as appropriate
Qualifications :
- Current licesure or certification as required by state regulations
- Current Licensed Practical Nurse or Registered Nurse
- 2+ years in case management or care coordination preferred
- Stron communication and interpersonal skills
- Abilitly to work effectively both independently and as part of a team
- Flexible with the ability to shift priorities when required
