Role Description
This is a full-time role for a Hospice Nurse Practitioner at Hospice East Bay located in Pleasant Hill, CA with weekend rotation.
The Palliative Care Nurse Practitioner (NP) is the primary clinician for PCEB. The Nurse Practitioner will make home visits, coordinate needed care, direct other members of the Palliative Care interdisciplinary team (IDT), create and update care plans as needed to carry out the patient’s / family’s goals of care.
Essential Functions :
- Attends palliative care consults as directed.
- Available as a consultant to patients, families and staff (hospital and hospice) regarding end of life issues and palliative care
- Assists with education and training of employees and community outreach.
- Adheres to the practice of confidentiality regarding patients, families, staff, and the organization.
- Assists with coordination and completion of Physician Orders for Live Sustaining Treatment (POLST) and Advance Directives.
- Coordinates patient care as directed by patient goals of care.
- Attends weekly Palliative Care team meetings.
- Serves as a member of the Interdisciplinary Team and participates in development of a comprehensive plan of care for patients served.
- Consults with the Chief Medical Director (CMO) as needed to ensure best practices.
- Documents in the Electronic Medical Record (EMR) indication for, and context of all patient encounters according to documentation standards as well as Medicare COPs.

- Mentors new NP hires for Palliative Care Program standards and integration into best practices.
- Participates with the hospice physicians or Quality Assessment Performance Improvement (QAPI) team on quality improvement projects as indicated or requested.
- Adhere s to the practice of confidentiality policies regarding patients, families, staff and the organization.
- Performs other duties as requested.
Position Requirements :
- Must possess a valid California driver’s license
- Must possess current auto insurance
- Excellent communication and interpersonal skills
- Demonstrated knowledge about home care, hospice, palliative care, advance directives, pain management, symptom control, and discharge planning
- A valid and current CPR certification required
- A valid and current BLS / ACLS certification required
Experience and Education :
- A valid and current California Nurse Practitioner license required
- A relevant Master’s degree required
- Active DEA Certificate for controlled substances (schedule II and higher) required
- Two or more years of palliative care experience with patients at end of life required
- Advanced Practice Certified Hospice and Palliative Nurse (ACHPN) preferred
- Minimum of 2 years Hospice and / or Home Care experience preferred
