Position Summary Functions in a leadership role to insure Social Work (SW) services are within regulatory requirements.
Serves as both functioning member of the CC / SW team as well as the Supervisor for Social Worker for SJ / C. Acts as a knowledge expert for the Social Work Team.
Participates in the ongoing and annual evaluation of the SW team. Works to nurture existing and develop new community s to Director Clinical Care Coordination
- Education Masters - Required
- Experience 8-10 Years Medical Social Work - Required Interpersonal and communication skills, ability to lead group meetings and initiate work flow changes.
Evidence of adaptability and willingness to adopt improvements.

Demonstrates a clear understanding of regulations applicable to patient care and / or department functions. Educate and precept social workers / social work interns to the role and related duties.
Acts as a lead resource person for other social workers for the purpose of providing direction and solutions to complex social issues.
Coordinates staffing needs for effective and efficient departmental operations. Guides, coaches and develops co-workers in order to facilitate the achievement of organizational, division, departmental and personal goals and objectives.
Annual co-worker performance evaluations, competencies and JCAHO requirements are reviewed annually with department staff.
Assess need for discussion with patient, family, physician and care team regarding patient's care or discharge plan. Schedule and lead patient care conference to resolve issues and provide clarification to patient, physician and family.
Seeks to identify gaps in health care services for targeted populations. Advocates at the national, state and local levels for their needs as identified through the work of special projects.
Discuss barriers with attending, if unsuccessful, escalate to department leadership.
