We are currently seeking a Case Manager for our Partial Care department. Partial Care provides treatment for the severely and persistently mentally ill population, who may also have substance abuse problems.
The program offers assessment and treatment planning, prevocational skill training, medication management, and case management.
The Case Manager Provides case management and support to PC clients. Facilitates groups. Documents according to regulatory standards.
Carries a caseload. Functions as part of PC Team. Interfaces with PC psychiatrist. Maintains collateral contacts with family members and providers in the community on behalf of clients.
