Perinatal Intensive Case Manager will provide a collaborative process between the pregnant mother and the Perinatal Center of Excellence utilizing the Nurse Navigator as the Coordinator of Care.
The Perinatal Case Manager will work with the Perinatal Center of Excellence team of physicians, Perinatal Certified Recovery Specialist, and Perinatal Nurse Navigator to provide supportive treatment and medical services for pregnant women.
Perinatal Case Manager will provide services throughout pregnancy continuing into the child’s 2nd year of age.
The Perinatal Case manager will facilitate linkage and access to available community resources and monitor retention in treatment and supportive services.
Additionally, the Perinatal Case Manager will educate the pregnant mother and community with the knowledge and skills necessary to achieve and maintain self-sufficiency, recovery and maintenance from substance abuse and opioid disorders.
Performs duties in support of the Medical Center mission to ensure the highest quality of client care in an economically sound and efficient manner.
This position serves : All pregnant clients and their children appropriate for Perinatal Center of Excellence Services.
Must be able to work a flexible schedule including some evenings and weekends
Education and Training :
Bachelor Degree in Social Sciences Preferred
Training and experience with Neonatal Absence Syndrome treatment protocols a plus.
Working knowledge of Community Services
Experience :
Bachelor Degree and One year experience indirect contact with mental health consumers.
High School Diploma and 12 semester hours in social sciences and Three years’ experience in public or private human services with One year in direct client contact with Substance abuse or mental health consumers.
Registration, Licensure and / or Certification :
Valid Motor Vehicle driver’s license.
