Job Summary :
A Caregiver supports the mission, operations, policies, and directives of Sages Elder Care LLC by providing direct support to clients as outlined in the Care Plan.
Caregivers seek to offer a safe and supportive environment for clients while being mindful of each client’s rights to direct their own care and each client’s right to self-determination.
Caregivers work with a care manager to follow and update a Care Plan that defines the individual request of clients for support in the home.
Pay Grade : Paid Per Hour
Job Qualifications :
Minimal Education : A high school graduate.
Minimum Experience : Prefer a minimum of five years of experience offering direct care for individual(s) who are homebound or infirmed or have requested support with activities of daily living.
Required to maintain a good driving record (no more than three current traffic
violations on DMV record) and to be able to pass drug, alcohol, and criminal
background check.
Must have proof of yearly Flu Vaccination and TB questionnaire every two years.
Skills
- Currently certified as a Personal Care Worker or licensed as a Certified Nursing Assistant or equivalent caregiving experience.
- Effective communication skills.
- Proficient use of a cell phone and / or other portable computer device.
- Able to read and follow a Care Plan.
- Able to offer client feedback and detailed client visit notation.
Physical Ability
- Able to stay awake and focused on client needs.
- Able to stand, walk, and / or sit for prolonged periods of time.
- Mobile and manual dexterity.
- Visual and Auditory acuity.
- Able to lift 2O lbs.
Working Conditions
- Exposure to risk related to home environments.
- Subject to unpredictable behavior and circumstances related to the care of elders.
- Able to maintain job performance in emergency or crisis situations.
- Able to offer support of clients in public settings.
Job Supervision : Caregivers will be under the supervision of a care manager.
Accountability and Performance Criteria
- Must follow Employee Agreement in relation to confidentiality, compliance with HIPPA standards, and Non-disclosure Agreement.
- Must demonstrate understanding of safety in relation to client / employee as outlined in Risk Management Policy, Abuse, and Neglect Policy, and as informed in personal care worker training and monthly in-service training.
- Must follow appearance, uniform, and conduct policies.
- Must demonstrate effective working relationship with care manager, clients, other employees, and people in public encountered during working hours.
- Must show up to client visits at scheduled time.
- Must demonstrate the initiative to offer helpful feedback verbally and / or in writing to enhance the client, employee, and organization’s effective delivery of care and services.
- Must attend paid in-service trainings and be available for supervision.
