This position will be based at the new and innovative acute care behavioral health facility, which opened in late 2023. SummitStone has the privilege of partnering closely with Larimer County on this venture.
This voter-approved facility provides licensed, equitable and culturally relevant care and treatment to individuals in crisis with mental health and / or substance use disorders.

- Status : Full-time, 40 / hrs. week
- Hours : Monday - Friday 8am - 5pm
What You'll Do :
This position is part care coordination and part operations. Someone in this role will need to provide care coordination services such as : tracking patient progress towards individualized goals, help maintain regulatory requirements for SUD treatment services, coordinate & collaborate with and across multi-disciplinary and multi-agency teams to serve clients across Larimer County.
Someone in this position will also build and maintain supply management processes and harm reduction supply distribution across the agency.
This position works side-by-side with Northern Colorado Health Network and Northern Colorado Health Alliance. Knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.
Duties include, but are not limited to :
- Provide care coordination services on behalf of and in collaboration with clients.
- Maintain and track supply ordering and distribution of Harm Reduction supplies.
- Engage with various stakeholders and community partners as needed to coordinate care including but not limited to emergency departments, health services, primary care settings, acute care systems, crisis response teams, and behavioral health teams.
- Outreach clients on a referral and / or cold outreach basis.
- Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network.
- Support clients navigating health systems and increase continuity of care.
- Participate as part of a multidisciplinary team and assist where needed within the scope of the position.
- Other duties as assigned.
Required :
- At least 1-year relevant experience in the Mental Health Field, Substance Use Disorder Field, or program of application.
- Experience with mild to severe mental illness and substance use disorders.
- Experience navigating systems of care.
- A desire to work with historically marginalized communities.
Preferred :
- Bachelors (BA / BS) Degree in psychology, social work or related field from accredited College or University. CAS, CAT, CAC III or LAC.
- Bilingual / bi-cultural preferred.

The typical hiring range for this position is $24.04-$28.28.85 / hour, based on relevant years of experience.
