We are a 120-bed not-for-profit nursing facility offering advanced healthcare while providing skilled nursing and rehabilitative services for both long and short term residents.
Located on eight beautiful landscaped acres, we also provide sub-acute services, wound care and respite services.

We are currently seeking an on call PTA who will embody the passion and commitment required to deliver the ultimate care for our residents.
Must be available to work Sundays as well as intermittent weekday coverage.
EDUCATION AND EXPERIENCE REQUIREMENTS :
Current New York State Licensure or eligibility.
Associate degree form an approved PTA program or licensure. Twoyears of experience as a Physical Therapy Assistant, preferably with 1 year geriatric
REQUIREMENTS INCLUDE :
Demonstrates the ability to discern the needs and behaviors of subacute / rehab patients appropriately, as they differ from those of the elderly
Addresses / accommodates the age-specific physical, psychosocialand emotional changes in residents / patients when planning programs and / or delivering care
Provides treatments to the resident in accordance with the goals and plan set by the Physical Therapist. Modifies or adjusts treatment within the limits of the plan of care based upon resident’s reactions.
Independently organizes a caseload that reflects the department’s productivity standard.
Maintains appropriate record keeping in accordance with facility and regulatory guidelines.

- Assists in the planning, development or revision of programs within the Rehabilitation Department.
- Participates in resident / family / discharge care planning conferences.
Performs other related duties as requested by the Supervisor
Assists in the transportation of residents as appropriate.
