QUALIFICATIONS :
- Must have at least one year of certifiable experience in home care, private-duty home care setting, community health agency, hospital or med-surg clinical rotation in a nursing school or have at lease one year of verifiable work experience in an unrelated field and if in an employed state where necessary, have successfully completed a Home Health Aide Training course approved by the Vice President of Home
- Demonstrate an ability to effectively communicate in English, and / or the predominate language of the population being services and must be able to read and
- Pass the Spring Hills Home Care Services Competency Evaluation
- Demonstrates competency in performing all
- Pass a criminal background
- Meet all medical requirements
- Meet all state
- Have reliable transportation and method of communication
- Is punctual, dependable and demonstrates sound
JOB RESPONSIBILITIES :
- Follows the established Plan of
- Assists with ""Activities of daily living"" means the functions or tasks for self-care which are performed either independently or with supervision or Activities of daily living include mobility, transferring, walking, grooming, bathing, dressing, undressing, eating, and toileting, per the Plan of Care.
- Assists with Independent Activities of daily living which may include light housekeeping, shopping, laundry and meal preparation, per the Plan of
- Accompany clients to appointments (transportation is client’s responsibility unless driving waiver is signed and per the Plan of Care)
- Provides assistance with self-administered, pre drawn and medication reminders, per Plan of
- Understands and maintains infection control and safety measures
- Reports any and all changes in the client’s condition to the Care Manager or
- Completes the Activity Records and accurately documents the care provided
- Performs all activities as directed by the Care Manager or BOM
- Has a positive relationship with clients and families
- Has positive client satisfactions
- Meets all deadlines for health screening requirements and
- Encourage maximum independence to facilitate rehabilitation as determined EP and
- Respect client’s rights and ensure confidentiality
- The CHHA / CNA / Home Care Aide job performance is evaluated 90 days into employment and yearly by Care Manager or BOM using input from clients, coordinators and personal
